treating ear infections
I know ear infections are common with infants, but after how many ear infections or at what level of severity should you ask to see an ear specialist? Also, are tubes in the ears the best route?
Ear infections are the most common bacterial infection in young children and account for a third of all office visits to pediatricians. Three quarters of all babies will have had at least one ear infection before reaching their first birthday. Pediatricians are well-equipped to diagnose, treat, and follow up most ear infections in children. The principle reasons to treat ear infections are to prevent infectious complications (spread of infection to nearby parts of the body) and to prevent hearing loss and associated speech or language delays.
No well-established criteria exist for referral of children with recurrent ear infections to an ear specialist (or otorhinolaryngologist, a surgeon who is an expert in operating on the ears, nose, and throat, hence "ENT" surgeon). The insertion of myringotomy tubes (small tubes put into the ear drums to equalize pressure between the middle ear and the outside) to prevent recurrent ear infections remains controversial. Prophylactic antibiotics (the use of long-term low doses of antibiotics) also have been used to treat children with recurrent ear infections. Some children have fewer ear infections following removal of their adenoids, infection-fighting tissue that lies between the back of the nose and the throat, that can swell with infections or allergies.
Fluid can remain in the middle ear, even after an acute infection has been successfully eliminated with antibiotic treatment. In most cases, the fluid will disappear spontaneously within 2-3 months. Children in whom the middle ear fluid persists and who have hearing loss because of the presence of fluid should see an ENT specialist to discuss myringotomy tubes.
Children with congenital anomalies that predispose to recurrent ear infections (such as cleft palate), children who have had a complication of an ear infection (facial nerve paralysis or mastoid infection for example), and children with chronic middle ear infection also should see an ENT specialist. All specialists are not alike, however; consult one with considerable experience in caring for children.
Keep in mind the role that you can play in preventing ear infections. Day care attendance (the larger the number of children, the higher the risk) and exposure to tobacco smoke increase the risk of ear infection; breast-feeding protects against ear infections.
